Terms, Conditions, and Liability Release

1. Eligibility: The drawing is open to individuals aged 18 and older, except employees, family members, and relatives of the business.

2. Entry Limit: One entry per person. Multiple entries from the same individual will be disqualified.

3. Entry Method: Entries must be submitted by using the entry form on Vitalisera.org by the deadline.

4. Prize: The prize is non-transferable and cannot be exchanged for cash or other services.

5. Winner Selection: The winner will be chosen randomly on November 22, 2024 and notified by email. 

6. Claiming the Prize: The winner must schedule a time to pick up the prize at 233 Cajon Street, Redlands, CA 92373, within [10 business days] of being notified. If the winner does not claim the prize within this period, an alternate winner may be selected.

7. Publicity: The winner agrees to allow their name and photo to be used for promotional purposes, if applicable. (We want to tell everyone who won!)

8. No Purchase Necessary: Making a purchase will not increase your chances of winning.

9. Right to Cancel: The business reserves the right to modify, suspend, or terminate the drawing for any reason.

10. Release of Liability: By participating in this drawing and accepting the gift basket, the winner acknowledges that the items included, such as exercise equipment and self-care products, are to be used at their own risk. The business, its employees, and affiliates are not responsible for any injuries, allergic reactions, or other health-related issues that may result from the use of these products. It is the responsibility of the winner to consult with a healthcare professional before using any items if they have any medical conditions or concerns. The winner agrees to hold [Vitalisera Institute of Pelvic Floor Physical Therapy, Inc.] harmless from any liability or claims arising from the use or misuse of the items included in the prize.
